Community Support Team Services

Our uniquely qualified staff teams work to support adults in the community, all in a coordinated effort to reduce or prevent hospitalizations and/or other institutional care. We work directly with the adult in their naturally occurring setting (home, school and community) on functional events that occur in those settings. Coordinators, Case Managers and therapists work directly with clients and provide First Responder services if there is a crisis in the home or community.
